Cant provision blueprint from VCO - java.lang.NullPointerException (Dynamic Script Module name : requestCatalogItemOnBehalfOf#1)

I am getting the above error.  Not sure why I am seeing this, but the logs show nothing.  The VCAC logs also show nothing.  %99 of my other blueprints are fine.  I can provision from VCAC without issue, but through VCO it is not working.
